We are looking for someone to join our Friendly team in the Fracture Liaison Service.

you will Take sole responsibility for coordinating all aspects of the Fracture Liaison Service administration.

Ensure that the long-term healthcare needs of patients with fragile bones, or at risk of falls, are identified and addressed.

Liaising and maintaining good relations with staff in Clinical Imaging, Rheumatology, Fracture Clinic, IT, audit department, Orthogeriatric Depts and external departments.

Independently solve problems and undertake analysis or investigation following up on enquires and obtaining information from other members of the team.

Lead the Fracture Liaison Service’s internal and external communications.

Responsible for identifying patients who have sustained a fragility fracture and have attended MIU, ED, fracture clinic or are an in-patient.

Responsible for data entry onto a database, to produce automated letters to be sent to the patients and GP.

Be responsible for systematic transfer of data onto an Excel spreadsheet and process it further to gather information about the patient’s compliance with medications, via questionnaires or phone calls.

Give advice to patients about taking their osteoporosis medication in the correct manner.

Responsibility on behalf of the osteoporosis team of ensuring accurate, timely data input to the national fracture liaison service database

Comprehensive knowledge and practical experience of hospital and IT systems including CRIS, PACS, PAS, MAXIMs, RADAR, E-referrals along with advance keyboard skills.

Good knowledge and understanding of both Trust and National Access Targets.

Ability to organise and prioritise own workload to meet all relevant deadlines.

Ability to deal with distressing situations.

Processing of referrals and all FLS appointments/cancellations on E-referrals system.

Sustainability is integral to the Trust achieving the NHS Net Zero target. All staff are therefore actively encouraged and supported to implement new ways of working within their field of expertise.

Compile stats to provide referral/appointment details to management and national FLS- Database.

Check and compile weekly data to ensure all FLS patients identified.

Escalating information to the Dexa service and FLS leads when appropriate.

Corresponding with patients regarding appointments, follow up and compliance of treatment.

To assist in the preparation and ensuring that Standard Operating Procedures (SOP) are followed, and which describe how the functions of this role will be carried out along with the timescales that will be adhered to.

Ensuring adherence to the Trust’s confidentiality policy

Liaising with secretaries, GP surgeries, consultants and other hospital departments, providing accurate and precise information and ensuring the accuracy of data recorded on the Trust’s PAS, E-referrals, CRIS and MAXIMs systems.

Communication will be verbal, electronic or in writing and may involve persuasion and tact.
