Band 7 Team Leader position

Are you a qualified District Nurse with the Specialist Practitioner Qualification (SPQ) or an experienced Community Nurse looking for an opportunity to join our Community Nursing team as a Team leader?

We have an opportunity for a Band 7 team leader post within Community Nursing. This role is to provide senior leadership to one of our four Community Nursing Teams and will provide an opportunity for you to develop your management and leadership skills.

Come and join one of our four Community Nursing teams based in the busy town of Swindon, with easy access to surrounding towns and Villages across Wiltshire and the beautiful countryside in adjoining Counties such as the Cotswolds.

You will need to demonstrate an ability to develop your skills in providing professional leadership and support to the team, whilst expanding these skills within the wider Community Team context, alongside other Band 7 team leaders. The provision of managerial and clinical support to junior members of the team, are essential as well as providing a supportive, nurturing environment for the team to excel in providing a quality service to our patients whilst developing their own professionalism and clinical skills.

**The post will require you to**: support the patient safety and quality agenda to embed a culture of excellent governance, develop processes for sharing learning from complaints and incidents, to effectively manage the team within agreed resources and to meet all financial targets.

You will need to effectively operate and manage this community team whilst working with other team leaders to ensure a consistent approach across Swindon. There is an expectation to undertake some clinical work in the community is part of this role.

1. To work in partnership with the local Commissioners, GPs and Social Care colleagues to ensure the teams deliver and support the effective flow of patients between primary community intermediate and secondary care.
2. Work closely with colleagues and lead transformational change whilst at the same time maintaining services and meeting all key deliveries.
3. To effectively operate and manage a community team..
4. Develop and maintain positive relationships with key stakeholders, including patients and the public all members of staff, local GPs and practice staff, colleagues from across the 3rd sector, commissioners and social care staff.
5. To provide excellent leadership for a community team, including all staff within the team.
6. Where indicated undertake clinical patient visits to support utilising specialised clinical skills and knowledge, in order to ensure safe and effective service provision, including clinical supervision to staff
7. To provide effective operational management of the team ensuring that resources available are utilized effectively and meet patient needs.
8. Ensure the flow of patients is timely and delays are mínimal.
9. Use data and soft intelligence to deliver and enhance performance meeting and surpassing all key performance indicators.
10. To be the local contact for GPs, acting in an account management capacity.
11. Contribute to the strategic planning including delivery of agreed business objectives.
12. To be an ambassador for community services
13. Provide monthly written reports (financial, activity, workforce, business, intelligence and transformation) including all identification of exceptions and solutions/actions.
14. Identify with colleagues opportunities for service developments and contribute to the delivery of business cases.
15. To propose policy changes and support implementation of policies within the Community setting.
16. Lead locally agreed service changes.
17. Support the patient safety and quality lead to embed a culture of excellent governance and develop processes for sharing learning from complaints and incidents.
18. Work with other community team leaders to ensure a consistent approach to delivery across the directorate.
19. To effectively manage the team within agreed resources.
20. To meet all financial and QIPP targets.
21. Meet all actions related to workforce and Quality targets including the delivery of CQUiNS.
22. Monitor the patient experience and constantly strive for improvement and positive experiences for all patients and their families.
23. Deliver the agreed agenda set out within the business plan.
24. To be put on the Community on call rota.
