- **Cornwall House Surgery JOB TITLE: Clinical Coder and Medical records summariser**:

- **REPORTS TO: PRACTICE MANAGER**:

- **HOURS: 20 hours/ week**:

- **Job summary: The practice is moving towards becoming a paper-light environment, and the main responsibilities of the post-holder will be to summarize and read code relevant information from patient’s clinical records and Docman correspondence into the practice computer system, working strictly in accordance with the practice notes summarizing protocol.**:

- **To be responsible for the SNOMED coding of relevant information into patients electronic medical records on the EMIS web clincial system. Code relevant QOF/IIF to support effective patient recall and to optimise income by ensuring the completeness of medical records.**:

- **Main duties/key tasks: Culling and sorting patient records and letters in preparation for summarizing. Reviewing medical records and producing an accurate summary of the patient’s medical history. Read coding the information into the practice clinical system. Auditing data collection standards in the practice monitoring progress of notes summarizing against practice targets, monitoring patient call and recall systems. Filing and retrieving paperwork. Streamlining the flow of clinical correspondence through the organization reducing the number of letters with a GP needs to process
