The successful applicant(s) will have the ability to undertake hybrid working.

The post holder would be part of an established team, that works closely with our Acute Partners, Adult Care and other multiple agencies.
- To provide a full and comprehensive administrative service in support of the Clinical Navigation Team including diary management, organising meetings, ordering and coordinating stock, and taking notes at informal team meetings and other associated events.
- To establish administrative systems and processes to maintain effective working of the clinical navigation team
- To ensure the accurate and detailed recording and processing of statistical data to provide the evidence required by the Trust, Integrated Care Board and National reporting platforms for monitoring and evaluation purposes
- To co-operate with the other administration staff in providing cover in the absence of colleagues as and when required
- Maintain constructive relationships with others both internal and external to Derbyshire Community Health Services Foundation Trust.
- May be party to sensitive and confidential information of a personal, financial and political nature and will therefore be relied upon to observe and comply with specific and implied confidentiality rules and procedure

The Clinical Navigation Team is mixed team of administration staff and clinicians who have a positive can do attitude which is promoted through the whole of the DCHS organisation.

We work closely together and with other discharge teams and have built up many positive cross teams working relationships.
