The post holder willact as the main focal point for visitors, relatives and callers to the ward; to include welcoming, dealing with face-to-face queries and defusing tense situation andbe responsible for dealing with all telephone queries, referring to the relevant departments as necessary. To respond to all correspondence.
- To ensure that the electronic bed state is kept up to date by entering and amending patient transfers and other details onto Systm1
- To ensure that all patient records are complete and in order, containing all the appropriate and relevant information and data, including all results from investigations to assist the staff e.g. search for x-rays
- When necessary liaise with family, Home Care Services or District Nurses, regarding a patient’s discharge to ensure that all necessary services have been arranged
- Sort and file pathology and X-ray reports into patients’ case notes after a member of the medical staff has seen them
- Deliver patients’ property and valuables to either the Patients’ Affairs Office or nominated next of kin for safe-keeping
- To ensure that all correspondence is sent out appropriately, including patient discharge letters to GPs and relevant information to the Social Services and the Care Management Team
- Make appointments for patients, arrange transport to other hospitals when needed and if necessary arrange nurse escorts
- To collect and file patient notes and x-rays and to track patient notes in and out of the ward
- To undertake or take part in administrative or multi-disciplinary audits as required.

West London NHS Trust is delighted to have been awarded the contract to provide community services in Ealing over the next 10 years. We have formed a partnership (Ealing CommunityPartners) with Centraland North WestLondonNHS Trust (CNWL), The Hillingdon Hospital NHS Trust and a number of other partners to provide community services throughout Ealing.

Intermediate care within Ealing consists of bedded rehab and admission avoidance, community admission avoidance, Discharge to assess Pathway one and Ealing Integrated Discharge Hub. Having this teams within one service allows us to support patient journeys effectively and support staff development.

The team is comprised of 183 members of staff and 38 contractors including, Medics, Nurses, Occupational therapists, Physiotherapists, SALT, Dieticians, Health care support staff, Psychologists, Admin and clerical and support teams inc. domestics and transport.

**Main responsibilities include**:

- Responsible for the day to day administration across intermediate with other members of the team
- Responsible for supporting colleagues ensuring work is completed within an agreed timeline
- Able to populate reports and collate data on agreed template
- This role will act as point of contact for patients and visitors, this will entail being calm, professional and empathetic whilst dealing with potentially sensitive and stressfully situations
- This role sits within Ealing Community Partners and will act as point of contact for peers and participate in peer support work
- The function of this role is IT based in line with the service objective of being paper light, this will entail fully utilising system one, Microsoft office (including excel and outlook) and other IT equipment such as telephonetics and projectors.
