An exciting opportunity has arisen within the Frail Elderly Assessment Team (FEAT) for an enthusiastic and motivated Registered Nurse to join our friendly, forward thinking team.

The FEAT Team successfully ensures local health services are always on hand for frail older people, putting their safety, care and experience at the forefront of the service we provide.

The team’s impressive impact on patient experience and bed utilisation, and the collaborative work with staff, will continue to improve the pathway for patients.

You will work within the Multi-disciplinary Team in the Emergency Department, Medical Assessment unit and short stay wards. You will assess patients and provide the 'gold-standard' for management of frailty-Comprehensive Geriatric Assessment.

You will work closely with Physiotherapists, Occupational Therapists and our prescribing pharmacists. We also have ACP's and Consultant Geriatricians in our team and you will work with them on a daily basis.

You will liaise with social services, care homes, the integrated discharge team and mental health services throughout the acute and community to ensure patients move effectively through the system and are discharged via the appropriate pathway.

You must have an excellent attitude to patient care; display professionalism at all times and be committed to the team ethos and development. Applicants should have good organisational skills and excellent communication skills are essential.
