**Position: Senior Care Giver**

**Summary of Position**
Ø To provide residents with an exceptional quality of life. We strive to promote a family culture within the home between residents and staff. As a Senior Care Giver your duty will be to promote our ethos at all times and to lead by example to the members of your team.
Ø All carers are expected to promote the homes’ values and principles at all times both during and outside of work.
Ø To act as Key Worker and adhere to the philosophy of person-centred care and observe the individualised care plans to meet each resident’s care needs and desires.
Ø To work closely with management and assist with the training and supervision of team members - promoting a positive culture, team work and monitoring performance.
**Duties and Responsibilities**
**QUALITY OF LIFE FOR RESIDENTS**
- To actively participate in ensuring our residents lead rich, happy and fulfilling lives at Pontcanna House.
- To promote and provide our residents with an exceptional quality of life at all times through activities they enjoy, based on your proactive approach to understanding their past, hobbies and choices.
- To ensure activities are organised and they are well managed on a daily basis
- To build a family-like relationship with residents.
- To ensure staff are fully aware of the needs of new residents prior to admission
- To assist in the admission of new residents and complete all necessary documentation.
- To assist completing the risk assessments and care plan within 48 hours of admission
- To ensure all staff provide our residents with their essential daily care needs and tasks - i.e. dressing, bathing, continence, mobility and eating.
- To assist residents where possible to help maintain and even improve their independence, mobility and individuality.
- To involve residents where possible to contribute towards their care plan and daily choices.
- To take care of residents clothing - monitoring to ensure the correct collection, distribution, care and return of all laundered items.
- To assist in keeping the residents’ rooms clean and tidy at all times along with the ancillary team. As a senior member of staff, you must monitor the cleanliness of the entire home ensuring a safe environment.
- To order, administer and return medication while following the homes’ policy and procedure to ensure privacy and dignity of residents are maintained. Also liaise with the GP, health care staff and pharmacy.
- Liaise with all medical professionals and involve where appropriate, in resident reviews
- Organise hospital appointments, district nurses and GP appointments and transport with escort.
- Record and communicate details of appointments to staff, families or representatives.
- Ensure a smooth handover of shifts, this must be communicated verbally and written.
- Supervise and organise shifts, allocate tasks to team members and communicate to staff, Monitor performance to ensure tasks are completed satisfactorily.
- Ensure the bath/shower rota is adhered to plus whenever any bath required per care plan.
- Ensure body maps are completed after bathing residents and when required to ensure the skin integrity is monitored regularly.
- When working nights, checking residents regularly throughout the night, ensuring their wellbeing and safety
**CUSTOMER SERVICE**
- To promote the excellent reputation of the Home at all times both during and outside of work hours.
- To be helpful and courteous to all relatives, visitors, and referring all enquiries about the residents’ care and well-being to the Care Manager.
- To be approachable, helpful, supportive and positive towards residents and other team members at all times to help contribute to the positive atmosphere of the home.
- To report and record all comments, compliments, concerns and complaints and follow up
- To communicate professionally to families/ representatives and update them all the time with resident’s health and wellbeing and their needs.
**SECURITY**
- To ensure the security of the residents’ property and the Homes’ property and premises. Report and hand in any lost property.
**HEALTH AND SAFETY**
- To understand and comply with the Homes’ Health and Safety policy.
- To carry out all duties in such a way as to ensure the economic and safe use of resources and equipment.
- To report all accidents, defects, damage, theft, breakages and hazards.
- To undertake risk assessments.
- To attend fire evacuation and all fire training sessions organised by the company.
- To carry out all duties in a safe manner, having regard to the health, safety and welfare of all employees, our residents, yourself and any other persons on the premises of the Home.
- To complete all the document related to health and safety.
