To support clinical services to understand their clinical effectiveness and to identify areas for improvement by ensuring information from sources including clinical audits, clinical outcomes and quality and effectiveness indicators, is available and understood.

To facilitate work that ensures clinical effectiveness aspects of quality are embedded across the organisation and operating effectively in order to support continual quality improvement.

Working with clinical teams to continuously reflect on the evidence base, to support teams to deliver care in line with the evidence base, and to support continual monitoring and evaluation of the effectiveness of care.

The clinical effectiveness facilitator will work across the Trust to:

- Help clinical services identify their evidence base and best practice guidance and support them to understand how this impacts, and possibly changes, service models and delivery of care.
- Support the identification of gaps in the evidence base and support services to monitor outcomes through audit, evaluation and data in order to generate further evidence.
- Encourage service-level ‘action orientated’ research, such as clinical audit and service evaluation, as an effective way for services to increase their evidence-base and to develop learning and improve outcomes.
- Embed systems for assessment and monitoring that demonstrate services are following the best available evidence and guidelines.
- Embed systems for continual audit and evaluation to assess the effectiveness of care and support the service to achieve the best possible outcomes for service users.
- Embed the use of outcome measures to support continual monitoring and improving services and care delivered.

The post-holder will take a key role in delivering organisational strategies, and the implementation of related improvement programmes; leading and co-leading innovative clinical effectiveness initiatives for the Trust.

The post-holder will work closely with the Clinical Effectiveness Manager to ensure effective systems are in place:

- To support clinical services to understand their clinical effectiveness and to identify areas for improvement by ensuring information is available and understood.
- To promote the development and realisation of an evidence-led culture, that brings together research, innovation, effectiveness and improvement to support change in clinical and operational practice.

The post-holder will work with key stakeholders (including Clinical Directorate leadership teams and local clinical teams, and corporate functions) to proactively identify, understand and support the improvement of clinical effectiveness aspects of quality.
