**Hours**:  40 - 7-week rota. 6 weeks 8am - 5pm Monday - Friday, 1 week Monday - Friday 11am - 8pm, some weekend work

Our client, a well-respected charity, provides support to improve the lives of people using their services by delivering a high quality and responsive support worker service to ensure a safe sound and supportive environment in conjunction with support workers and the service manager

**Synopsis of duties**:

- Supporting a caseload of clients that are homeless/rough sleepers
- Experience of completing Needs and Risk Assessments, Support Plans, and knowledge of working with homeless, vulnerable client groups
- Facilitate client meetings and other client participation methods
- Conduct regular client support plan reviews, completing and updating individual support plans; within contractual timeframes and work with clients to achieve their identified goals in accordance with policy, including client benefits being maximised and personal payments are made as part of a budget plan
- Provide relevant and suitable options of external support provision, when identified support needs cannot be met through in-house provision
- Complete and review client’s assessments including risk assessments, within contractual timeframes, using a person-centred and trauma sensitive approach
- Utilise the bespoke web-based client management system ATLAS, to accurately record all relevant data in a timely manner
- Respect the diversity of other people’s culture, faith and practice; ensuring compliance with the Equality and Diversity policy
- Ensure all practice is in line with relevant legislation, guidelines and polices to protect clients, colleagues, staff and visitors from any abuse, accident or injury
- Provide evidence to show that contractual KPIs and quality objectives are achieved
- Ensure there is good communication with colleagues so all those working with the clients have information to assist with achieving the client’s goal

**Essential Requirements**:

- Proven track record of supporting vulnerable young people and/ or adults either within an accommodation or community-based service. Preferred experience of working with addictions, substance misuse, homelessness or mental health
- Strong safeguarding and risk assessments skills and experience
- Proven track record of successfully carrying and managing your own caseload
- Enhanced DBS issued within the last 12 months and/ or registered on the DBS Update Service
