We have an exciting new opportunity to join our organization and help support our Co-ordination center as a Clinical Assessor.

The Co-ordination Hub is a seven-day service, with the aim of supporting patients in their own place of residence where it is clinically safe to do so, reducing the number of unnecessary attendances to the Emergency Department and unplanned admissions.

This service is a new innovation, therefore we need additional staff to offer greater support to the new innovations in our work using telemedicine.

You will be assessing patients over the phone with GPs, Paramedics, ACPs, Community Nurses and other Clinicians to determine the best pathway for the patient. You will have support and training for this.

The Clinical Assessors take calls from Care Homes across BSW to support them with decisions regarding the correct pathway for their residents to prevent a hospital admission.
