We are looking for an enthusiastic and proactive individual to lead and develop the Trauma ESD (Early Supported Discharge) service within our Trauma and Orthopaedic therapy team at Gloucestershire Hospitals NHS Foundation Trust, ensuring an effective and evidence based service through assessing, planning and implementing treatment programmes. The post holder will ensure comprehensive assessments are completed, with a ‘home first' approach to discharge planning. Positive risk taking will support safe and early discharge, with robust follow up service delivery in the patients own home.

You will have the opportunity to work with trauma and medical patients on the wards supporting assessment, discharge planning and rehabilitation, following these patients up in their own home to support them back to independence.

The successful applicant will join our experienced integrated therapy team, working alongside occupational therapists, physiotherapists, therapy support workers and the wider multidisciplinary team.

- disciplinary team is essential. You will need to have excellent communication skills, have a positive attitude, compassionate, and be able to work as part of a team. Being able to motivate and encourage patients in an active way to regain and maintain their independence is important.

The post holder will be required to use the principles of Occupational Therapy and Physiotherapy in an integrated way to promote independence and facilitate safe discharge. As a senior decision maker, they will take the lead for managerial and operational responsibility for the trauma ESD service, using evidence based practice to support patients on discharge, aiming to promote independence within the patients own environment.
Working with a home first approach, the role will support and enable patient discharge home at an earlier stage in their rehabilitation, providing support in the home environment to achieve patient independence.

To work as an expert practitioner in Trauma &Orthopaedics, utilising highly specialist theoretical knowledge, clinical experience and advanced clinical reasoning to provide comprehensive patient management. This will include patients with diverse or complex presentations and those referred from other therapists and health care professionals for a second opinion/advice on further management.

To be responsible for operational management of the ESD therapy service by monitoring, coordinating own and other caseloads, including reallocation of work during absence and appropriately delegation to junior therapists and support workers.

To exercise a high degree of professional autonomy, creating and developing protocols/guidelines for the management of patients. To make recommendations regarding national guidelines, initiatives and research. Lead best practice by the use of validated assessment tools and review of intervention outcomes in order to ensure a high-quality evidence-based service is provided.

To promote a Home First approach to discharge planning to facilitate patient flow, working closely with community partners and the wider MDT

To provide clinical expertise, education, support and guidance for therapy staff, undergraduate students and junior medical staff in order to develop clinical skills and to promote consistency of practice trust wide. To promote effective communication with therapy, consultant, nursing and other healthcare staff in order to agree an inter-professional approach to patient management and ensure equity of care.

To maintain clear, accurate and concise patient records in line with departmental and professional standards and include relevant outcome measures. This will include writing patient reports for e.g., medical practitioners and solicitor’s requests.

To be responsible for monitoring and evaluating the performance of the staff working within the speciality and taking action as necessary to ensure a high standard of evidence based clinical practice and patient safety, in order to fulfil clinical governance requirements.
