There are some really exciting things happening in both Sutton and Kingston. We are developing the London Cancer Hub, a world leading cancer research and treatment hub that will bring together professionals from across different sectors to identify and test new treatments and therapies. We are undertaking the largest housing regeneration programme ever delivered in Kingston, building new energy efficient housing that will transform some of our oldest Council owned housing. Kingston is a beautiful Royal borough with a historic setting on the banks of the Thames with excellent schools and a bustling retail centre, all surrounded by open green spaces such as RIchmond Park and Bushey Park, former recreation grounds for Tudor kings and queens. **About the Role**

We're recruiting for 1 x full-time post (36 hours per week) on a 2 year fixed term contract. Whilst this is a hybrid role there may be requirements to work from either boroughs head office sites, Civic Centre in Sutton and Guildhall buildings in Kingston.

This role will be responsible for oversight and delivery of all Disabled Facilities Grants (DFG) ensuring services are delivered efficiently, they will also be expected to:

- Lead on the design and definition of outcomes, setting targets and monitoring of performance with managers and colleagues within a culture of continual improvement
- Be responsible for ensuring the regular collection of performance indicators, and ensure the timely completion and submission of any statutory returns by the joint Home Improvement Agency.
- Support and actively contribute to operational plans to ensure that the resources within the teams are used to best effect and positive impact for our residents
- Work with multi-agency professional services (Occupational Therapists, Hospital Social Work Teams, wider Adult Social Care operational groups, Health partners, Housing, the Home Improvement Agency, and other service providers
- Perform and manage the case coordination function to prevent hospital discharge and facilitate early discharge from hospital and other settings.
