Act as a specialist clinician in a specific clinical area, participating as a member of the team independently managing a specialist clinical caseload.Provide high quality physiotherapy assessment and treatment to adults referred to the CNRT.Develop and deliver an individualised treatment program to the clients referred. Work closely with the multi-agency team in order to meet the needs of the clients / patients and enable them to receive a comprehensive service.

Be responsible for a defined area of the service or a particular patient type, working without direct supervision.Supervision takes the form of regular formal training, clinical reasoning sessions and peer review. Access to advice and support from a senior physiotherapist is available if required, clinical work is not routinely evaluated.

Assist in the running and development of group exercise and education programs.
- Be professionally and legally accountable for all aspects of own work including the management of patients who are under own care.
- Develop comprehensive discharge planning in collaboration with the multidisciplinary team.
- Be responsible for providing timely quality management of the caseload
- Respect the individuality, values, cultural and religious diversity of clients and contribute to provision of a service sensitive to these needs.
- Progress the client intervention to a satisfactory closure, evaluating outcomes, setting up reviews and/or arranging appropriate transfer of care as appropriate.
- Actively involved within the multi-disciplinary team and participate in relevant areas i.e. clinics and effectively communicate to contribute with other Health Care Professionals in the planning of patient care.
- Work with a band 5 therapist, rehab assistants and technical instructors regarding clinical caseload and provide necessary support on clinical issues.
- Ensure that all documentation around patient care is recorded according to Trust and CSP Standards of Practice.
- Provide high quality safe care with compassion. Listen, and communicate clearly at all times. Demonstrate dignity and respect to all patients and colleagues.
- Exercise good personal time management, punctuality and reliability.
- Participate in the delivery of the service business and clinical governance plans including any relevant duties as dictated by service needs.

Hounslow and Richmond Community Healthcare NHS Trust (HRCH) provide a wide range of healthcare services for the population of London Borough of Richmond upon Thames and the London Borough of Hounslow.

The Clinical Services Directorates have a wide range of responsibilities to manage, including co-ordinating and delivering community primary care services on behalf of the Trust. A wide range of these healthcare services are delivered by staff in the community, who include medical staff, paediatric therapists, therapy assistants, health visitors, specialist nurses, nurses, nursery nurses, paediatric phlebotomists and healthcare support workers with the support of administrative staff working in localities with GPs, other health professionals, early years, education, and social care professionals.

The digitalisation of health services is underway with a requirement to become paper-light by 2023 through improving the use of digital tools and adopting mobile and agile working practices and increasing the efficiency and effectiveness of clinical delivery.
