**Locum Band 6 Community Rehab Physiotherapist - Berkshire**

We have a fantastic opportunity for a locum Physiotherapist to join a leading Community hospital, based in Berkshire. They are seeking a qualified Physiotherapist with previous Community experience.
- Enhanced DBS
- Desirable - postural and contracture management.
- Community Rehab - physiotherapy exercise programmes, strength, balance, transfers, mobility, intensive input for ICR patients post hospital discharge.
- Identify patients who meet ICR criteria and coordinate ICR discharge once patients meet criteria
- Integrated working between Acute / Community services / Social care/ voluntary sector
- To holistically support the patient throughout the rehabilitation process and to have a patient centred approach
- To ensure that patients have a clear rehabilitation plan identifying their needs and ensure appropriate actions are taken prior to discharge from Acute hospital
- To undertake risk assessments and implement risk reduction measures in line with current policies and guidelines
- To assist the patient in identifying their goals and to recognise when these have been achieved.
- To liaise withIN MDT
- To work as part of PACE Team (Post-Acute Care Enablement) and supporting team as required for smooth running of service.

**Position Requirements**
- HCPC Registered
- 3 month (minimum) locum assignment to start as soon as possible
- Full time hours (Monday - Friday)
- Minimum of two years postgraduate experience
- Previous clinical experience performing a similar role within a Community setting
