CLCH Hertfordshire Adult Community Services -CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting

An exciting opportunity has risen to work in our bed-based unit at Potters Bar Community Hospital. The ward is a 29 bedded rehabilitation unit working as part of Herts Division Bedded Unit for Central London Community Healthcare.

We are looking for a dynamic Band 6 Registered Nurse with experience in acute or community hospital.

The successful applicant will need to be able to demonstrate trust values and have a passion for working with patients during their rehabilitation enabling them to regain maximum self-sufficiency and return home. Applicants must be able to exhibit leadership, motivate and support staff towards continuous learning in the work environment.

You need to be active, innovative, diligent and possess excellent written and verbal communication skills.

- To be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care
- To work in a collaborative manner and liaise with other professionals and agencies to ensure a high quality service for patients
- To lead and monitor the performance and work of the nursing team

- To ensure the effective running of the Ward in the absence of the Ward Manager.
