Job Introduction
We are looking for a **Team Leader** to work in our Urgent Community Response Service in West Lancashire. The team ensures that crisis response care is available to all people within their homes or usual place of residence, including care/residential homes.

The role will be a **Full Time, Band 7** role with **Agenda for Change** terms and conditions including NHS Pension. The service is open **365 days** a year is operational between the hours of **8am - 8pm.**

You will work with the Head of Service to continually review, develop, and refine pathways of care; facilitating the delivery of a robust, innovative, and expert service and you will support all members of the team with the development of professional standards

Main Responsibilities
Responsibilities include but are not limited to:

- To be responsible for the daily operational leadership of Urgent Community Response Team, escalating actual or potential risks to clinical, financial, or operational standards to the Head of Service
- To maintain an up-to-date knowledge of national guidelines and legislation from the Department of Health and other relevant services and to be aware of their impact on the profession in relation to evidence-based practice
- To encourage team input and contribution to new ways of working and the modernisation of nursing / public health agenda
- Provide effective skilled communication of sensitive issues, counselling, and reassurance to maximise patient and carer wellbeing
- To be able to deal with complex situations which require analysis and comparisons of a range of options on a daily basis. To be able to make judgements on a variety of therapy related problems which require investigation, analysis, and assessment

We are looking for someone who can act as a positive role model and be visible to your team, demonstrating effective communication, leadership styles and behaviours. You will be able to work well in stressful situations whilst showing understanding and compassion.

You will have previous experience in a Band 7 role, working in a team effectively. The ability to make clinical decisions associated with high clinical risk and potential harm to patients.

A full UK driving licence is required for this role and access to a vehicle.
