Band 6 Dietician

We have an exciting opportunity for a Qualified band 6 Dietician to be part of our integrated health and social care team based at Halton. We have been on an exciting journey to transform the intermediate care services within Halton and have brought together health and social care into a unified integrated team.

We are looking for a well organised individual with a passion for promoting independence to join our friendly team to work across all areas of the service including:
Single Point of Access

Community Rapid Response / Frailty Team

Reablement Team

Intermediate Care Unit

You will work as part of a large multidisciplinary team that includes GP’s, Advanced Nurse Practitioners, Physiotherapists, Occupational Therapists, Social Workers, Therapy and Nursing Assistants, Care Support Workers, and Administrators.

The Service operates a 7-day service, currently covering the hours of 8am to 8pm, including bank holidays and weekends. You will be expected to work shifts to cover these hours.

To work collaboratively with health and social care professionals to undertake proactive screening of all referrals into Single point of access, establish priorities and correct pathways within agreed protocols.

To work to a high standard as an autonomous Dietitian.

To provide a range of specialist assessments, diagnosis and dietary treatments to patients presenting with a variety of conditions. The role will manage the complex community caseload, with great opportunities to seek further development with the support of the Band 7 staff.

A full UK driving licence and access to a vehicle is essential for these jobs.
