Summary

We have an opportunity for three Prevention and Wellbeing Officers to join our Prevention and Wellbeing Team Hubs in Trowbridge, Chippenham and Salisbury.

**LIVE **WORK** SUPPORT**

Using a strengths based approach, our Prevention and Wellbeing Team work with people in the community to develop and strengthen their networks and engage with community-based support.

**About the role**

You will work with people who experience challenges in their lives, with a focus on promoting their independence and reducing the need for formal support, where appropriate. Some of those people’s challenges and situations will be complex and there will be a need to support with the management of risk and to work with a range of different organisations.

The team will work both with people who do not have formal support and with those who do, to ensure that support plans are put in place to maximise people’s strengths and opportunities to access community-based support and make positive changes to their own lives and future aspirations.

**About You**

We will look for you to have previous experience of working in a health or social care environment or in a similar setting, with experience of working with people with complex needs, providing support and encouraging independence.

You should have excellent interpersonal skills and the ability to effectively communicate with people from all walks of life.

You will have good organisational skills, be able to prioritise and manage your own workload to meet deadlines, work without direct supervision and know when to discuss with supervisor/manager.
