**The Role - Care Coordinator (Full time permanent Position)**

The Care Coordinator / Scheduler is expected to perform a variety of duties in the coordination of scheduling visits to our Clients in their own homes.

The Care Coordinator is responsible for scheduling our Clients and Caregivers in order to provide the highest quality service to our Clients with emphasis on creating extraordinary relationships.

Our Caregivers are matched with our Clients and visit their Client on a consistent basis. This means that the majority of day to day scheduling is covering sickness and holiday because the majority of visits are already planned as regular shifts.

The Care Coordinator will also be responsible for planning new Client schedules and matching Caregivers accordingly. The Coordinator will be speaking to Clients and their families on the phone organising extra shifts, cancelling visits and discussing any changes to the regular schedule.

Provide timely response to client requests and ensure they are constantly updated with new and changing information
- **Quality Assurance - **Your role will be integral to the quality assurance of our service and will include introducing Caregivers to Clients and being included in the visits and Service Reviews to our clients that ensure we support changes in their needs and Care Plan updates. This is a vital element of our service as each Care Coordinator meets the clients and caregivers that they are scheduling for.

**Person Specifications**

You will have proven experience in the care sector and be able to develop excellent relationships with both clients and caregivers, creating client schedules and records and supporting the team by working with families, professionals and other caregivers to achieve the very best outcomes.

You will ideally be able to hit the ground running - Have sound knowledge and understanding of schedule planning software systems, allocating and coordinating all care visits to ensure continuity for both client and caregivers
- Be adept in organising and communicating caregiver / client rotas - Be able to adapt quickly to emergencies and / or other unforeseen circumstances ensuring all visits always take place
- An outstanding eye for detail and the ability to multi-task and prioritise conflicting deadlines.

To have pride in your work; to want to work ‘above and beyond’ and have high standards of commitment and achievement. Good understanding of Microsoft Office programs. Confidence in using a computer. Well organised and ability to plan ahead. Ability to work by oneself as well as part of a team. Detail orientated. Focused, proven problem-solving skills. Robust and resilient under pressure
