Clyde Sector are delighted to announce the opportunity to join the Vale of Leven Older People’s Service. The Frailty Practitioner post is a leader in supporting those with frailty from admission to discharge in the VOL site. The post holder will be autonomous practitioner working across all specialties and will work across organisational boundaries to support the Older People teams, liaise with social care and health services to provide comprehensive geriatric assessment and optimise patient outcomes.
