**Change Grow Live** you will based primarily at Kings Mill Hospital in ED and admissions wards and will provide rapid access to assessment, brief intervention and liaison, discharge planning and onward referral for service users attending hospital with substance misuse related issues.

**About the role**:

- To provide specialist assessment of patients attending/admitted to hospital with alcohol related issues using AUDIT C and AUDIT 10 screening tools
- Provide advice, support and training to hospital staff regarding drug and alcohol awareness, screening, assessment processes and referral pathways
- Work with community teams to ensure assertive and effective follow up of both repeat presentations and discharges from the hospital
- Develop specific links with identified specialist acute services and provide in-reach to hospital wards including: Gastroenterology/Hepatology; Orthopaedic[1]falls prevention services; specialist clinics (i.e.: Hypertensive Disease); observation ward; ED

**About you**:

- Training and/or experience in psychosocial intervention
- Have the ability to build working relationships and communicate with all members of the Emergency MDT
- Ability to manage challenging behaviour in a calm and professional manner
- Sound working knowledge of safeguarding
