Main Duties and Responsibilities: Support the delivery, of the county wide model of individualised cardiac rehabilitation programmes, adapting their exercise prescriptions to meet individual needs. Work in partnership with the specialist nurses and exercise instructors in supporting patients to achieve their tailor made, person centred goals reviewing and adjusting accordingly. To help with the delivery of the physical activity component of the programme.

To assist with sub-maximal exercise assessments for a range of cardiac conditions. To work as part of a team to directly deliver and promote physical activity and lifestyle interventions to adults within community setting as required. To visit patients in their own home and in clinic settings working without direct supervision on occasion and reporting any clinical findings to the specialist nursing team. Accurately record and document the delivery of care.

To effectively plan, prioritise and manage own workload. Maintain equipment and resources, required to support physical activity programmes. Participate in the effective reporting of feedback, incidents and complaints. This includes submission of appropriate documentation to facilitate the investigation process.
