Take a lead role in the advanced assessment, treatment and management of adult patients with complex needs referred from the acute surgical wards; including gastroenterology, general surgery and bariatric surgery patients. Contribute to the management of patients on the critical care unit. Be responsible for the management of gastro outpatient appointments including facilitation of FODMAP education group sessions. Provide cover for the management of patients diagnosed with a diagnosed eating disorder as required.

Use advanced clinical reasoning skills to set nutritional goals for each patient. Correctly interpret complex nutritional impairments, biochemical parameters and fluid balance. This will involve nutritional diagnosis including interpretation of biochemistry, anthropometrics, clinical condition, social factors and diet histories, in patients with highly specialised and complex nutritional needs. Make clinical decisions as part of the MDT about the appropriate forms of artificial nutrition support and calculate patients nutritional requirements and formulate suitable enteral feeding regimes.

Frequent monitoring, review and adjustments of feeding regimes as necessary. Undertake the supervision and assist with the organisational management of junior staff, assistants and students. Be responsible for the supervision and assessment of students during their placement. This will include development of tutorials, assessment of work and planning workloads for the week.

Contribute and assist with organising and delivering the department's CPD programme in collaboration with other senior staff. Maintain own professional development and demonstrate high standards of clinical performance and professional practice. Work as an autonomous practitioner including being responsible for planning and prioritising your own workload. Undertake in-service training relevant to the post.

Undertake audit as part an integral part of a workload and to ensure guidelines relating to your specialist area are fully implemented across your area of responsibility. Be a core member of the multidisciplinary care team, liaising with other members of the team to provide integrated dietetic care. Plan and implement effective and appropriate training for dietitians, medical and nursing staff, carers, members of the voluntary sector, AHP colleagues and others. Be responsible for maintaining accurate and comprehensive patient records in line with BDA, Trust and department standards.

Develop diet sheets and other resources for patients and health professionals, as appropriate both within the department and the wider health community. Use audit/evidence-based practice/research to help develop improvements in service delivery and clinical practice discussing recommendations for change with the team lead and to implement them as agreed. Be flexible in day-to-day work patterns to best suit patient needs. Participate in the department appraisal scheme as an appraisee and be responsible for complying with agreed Personal Development Plans to meet knowledge and competencies.

Maintain accurate, comprehensive and up-to-date documentation, in line with legal and departmental requirements, communicating assessment and treatment results to the appropriate disciplines. Maintain the confidentiality of the Trust in respect of patient and staff information obtained at all times, and use such information only as authorised for specific purposes. Report any concerns about the use of such information to the senior manager. Be responsible for safe discharge planning, including co-ordination of discharge of patients on home enteral feeding.

Communicate highly complex specialised and sensitive information with empathy and understanding. Liaise with the catering department to ensure provision of suitable meals for patients on therapeutic diets. Be professionally and legally responsible and accountable for all aspects of own work, including the management of patients in your care. Be involved in the development of nutrition policies and guidelines for people with gastroenterology conditions and pre/post surgical recovery.

Have a detailed knowledge of national and local standards and the current evidence base in nutrition and a wide range of general, gastroenterology and surgical dietetics. Accept clinical responsibility for a designated caseload of patients, and organise this effectively and efficiently with regard to clinical priorities and use of time. Collaborate on patient joint care with dietitians across hospitals in the network on a regular basis. Provide cover to other wards/departments at short notice to cover unplanned and planned absences within the post holders competency and skill level.
