Assess patients’ holistic needs, communicating complex and sensitive information to patients and carers as to their assessment.

Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in
order to gain understanding, consent and concordance.

Be required to use tact and persuasive skills in order to gain the patient’s co-operation in their care management plan.

Be aware of assistive technology and utilise where appropriate.ie (Luscii, Docobo).

The post holder will be expected to utilise technology to support the role, including undertaking online and video consultations with patients.

Ensure patient held records are completed for each point of contact and inputting escalation on Clinical Web Portal on a daily basis.

To assist the nursing staff with direct care of patients, working regularly without direct supervision but with guidance from a registered nurse.

To keep the clinical environment tidy and well maintained.

To provide support to the nursing team which will contribute positively to the continuing development of the clinical service.

To assist registered nurses* with care of patients, working regularly without direct supervision.
To ensure the set standards of care are maintained for all patients.
To virtually support patients via telephone, video or technological (App).
To accurately record and report observations e.g. pulse, respiration, blood pressure and temperature with the aid of specialist equipment as appropriate to the needs of the work place.
To provide education and health promotion Making Every Contact Count.
To assist in health education or information giving as indicated by the registered nurse.
To ensure that appropriate moving and handling techniques and equipment are used when manual handling.
To feedback information gathered from patient interaction, plus care given, to registered nurse* entering relevant information into the care records as necessary. All entries are to be countersigned by a registered nurse.
To answer the telephone and take messages as required.
To ensure all areas are kept clean, tidy and appropriately stocked, including storage areas and medical equipment.
To attend mandatory training and such courses/lectures/in-service training, as indicated by the needs of the service and own personal development as identified at appraisal and within personal development plan.

To alter communication methods in order to effectively communicate with a child or young person and their family.
