Dementia Care Coordinator

We have an opportunity for a full-time position (35 hours) providing pre-diagnosis support in GP surgeries in the Pocklington area.

Dementia Care Coordinators work within primary care to support people to get a diagnosis and ensure they receive the right support at the right time. Whilst the main work is carried out within GP surgeries the role will be embedded into the wider Dementia Forward team of Dementia Support Advisors and Helpline worker

We are looking for someone with passion and drive and the ability to empathise with people affected by dementia.

Knowledge and experience of the condition and the health and social care system would be an advantage.

**Salary**: £24,133.20 per year
