**Continuity of Care Coordinator**

**About the role**
We are currently recruiting for a Continuity of Care Coordinator to join our Recovery Coordination teams based in Co Durham. Our Team’s provide support for individuals who are experiencing problematic substance use.
- The Continuity of Care Coordinator role will ensure that we are able to provide continuity of care to prisoners leaving custody and re-engaging into the community. We provide specialist, tailored support to enable service users to make positive changes, which affords us the knowledge and resources to be able to reach and engage people with a range of needs within our communities. The main duties of the role include:

- Working collaboratively alongside Prison and Probation colleagues.
- You will support individuals who require additional intensive support to engage with treatment services.
- You will provide regular liaison with Prison DART Teams and facilitate potential in-reach and video link assessment, prior to community release.
- You will offer tailored guidance in minimising harmful behaviour.
- If required, support those into service should they need more substantial support.
- You will take a “whole person” holistic approach, ensuring that every service user’s package of care encompasses all areas of their wellbeing, not just their substance use, to achieve the best possible outcomes for every person.
