National and Regional teams have been working with systems to develop the Virtual Wards programme of work and associated guidance.

Shropshire, Telford and Wrekin have been set the target of delivering 250 virtual ward beds by December 2023. We are currently devising our plans detailing how we will do this and are now looking to recruit a team of experienced and expert clinicians and leaders to deliver this exciting and innovative service.

We are looking to recruit a range of Advanced Clinical Practitioners, Pathway Coordinators and Clinicians that can lead on the implementation of digital solutions and remote monitoring. There must also be an appetite and enthusiasm for any applicant to be willing to explore and test new ways of working that will enable us to maximise the offer and support of the virtual ward to all members of our communities.

The post holder must be able to form key working partnerships across health and social care, working flexibly and creatively ensuring that the patient remains at the centre of their clinical and therapeutic interventions. A philosophy of patient choice being at the heart of decision making is also key to this programme of work. There might be a requirement at times to work across different pathways within the community to ensure that there is continuity of care, and that capacity is managed where there is a greater clinical need.
