An opportunity has arisen in Cancer Services to support the delivery of the national Cancer Waiting Times objective.

The successful applicant will be responsible for coordinating MDT meetings, including prepping the meeting list, live minute taking and ensuring that outcomes are recorded and actions completed in a timely manner. The applicant will be responsible for ensuring that patients are discussed at MDT and treated in line with national targets. The applicant will also be responsible for the timely collection of key data items in line with national audit dataset requirements.

The applicant will establish key working relationships with all members of the MDT, including administrative and clinical teams. They will act as a point of contact in communications with patients, clinicians and other external hospital staff /tertiary referrers.

The role is required to work closely with the operational services to ensure that patient pathways are expedited as required.

The role involves using multiple hospital systems, including PAS, ICE and Somerset.

The post holder is expected to cover other team members during periods of absence and increased activity.

The core purpose of the MDT co-ordinator is to provide consistent patient centred focus throughout the pathway of care, by providing a point of contact along the administration pathway for the MultiDisciplinary Teams (MDTs). This will ensure coordination of the patient journey, especially for patients that need to move between cancer units and cancer centres. The MDT Coordinator, with the support of an assigned Cancer Pathway Tracker (where applicable site dependant), will be responsible for
tracking the patients along their care pathway and identifying potential breaches of cancer waiting times and targets

The role will ensure that all relevant patients are discussed at MDT meetings and all supporting clinical information is made available. Also accurate prescriptive data collection and recording to enable effective patient tracking of all suspected and positively diagnosed cancer patients
The post holder will be responsible for the coordination and organisation of the multi-disciplinary team meetings and will attend these meetings obtaining and recording relevant information. He/she will
collect, record and report cancer information as required in order to meet both national and local requirements

To monitor patient journey against cancer waiting time targets, ensuring that each patient pathway is monitored against agreed pathways and in accordance with the Trusts Escalation Policy

Ensure all patients are discussed at MDT in a timely manner with all relevant information (imaging/ pathology etc) available to enable informed treatment decisions

Coordinate MDT meetings, including prepping of meeting list, live minute taking and ensuring that outcomes are recorded and actions completed in a timely manner

Maintain robust data collection systems ensuring that information is collected in a timely fashion and entered onto the cancer database

Monitoring and amending data input to ensure accuracy

To liaise with diagnostic departments and clinical specialty staff across the Trust regarding patient appointment and care plans to ensure compliance with all agreed timescales

To highlight potential breaches to targets as appropriate and identify appointments to be brought forward

To ensure that all data is accurately recorded on Somerset to ensure that patients are fully discussed at MDT, to include investigation reports and pathology reports

To participate in the weekly cancer PTL performance meetings and follow up any agreed actions
