Social Care Assessor

An exciting opportunity as arisen in the Devon Onward Care team where you can be at the forefront of supporting people to return to the community with complex care needs. You will be responsible for facilitating discharges from Kingsbridge Community Hospital supporting service users to safely return home or care home setting.

We have adopted the Discharge to Assess Model which is facilitating complex discharges from the community hospital. You will be based in a Kingsbridge community hospital setting working collaboratively with our hospital and community-based colleagues and within a whole integrated system.

The role would provide an excellent opportunity for anyone who is looking to progress their social care career. You will be working alongside a health and social care team which comprises of Discharge Nurses, Senior Social Workers, and other Social Care Assessors. You will be working in partnership with the wider Community Health and Social Care Team within the Discharge to Assess model.
