Our General Acute Medicine and Frailty Virtual Ward is a new and innovative Service, which has an agreed admission criteria and works in partnership with supporting care services to deliver acute care in the patient’s own home. The aim of a virtual ward is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients’, and their carers’ lives. This means we can proactively manage people living with frailty at home, and for those that have a sub-acute episode we can improve a patient’s experience and outcomes. In addition, this means we have more space in our hospitals for acutely unwell patients and reduces the pressure on urgent care.

The main priorities will be to lead the Case Management and Bed Management of the Virtual Ward. Management of Virtual Ward staff including the management of the Virtual Ward Clinical Lead and the Advanced Clinical Practitioner (ACP) will be pivotal to the success of this service

The Matron role is expected to be part of an on call rota with the other Senior Managers

It is essential that all applicants must be able to drive and have access to a car for business use.

The role is responsible for the case management of the South west up to 134 virtual ward beds.

The Service is managed over 7 days per week 8am-10pm and the role is expected to part of an on call rota with the other Senior Managers; Working in partnership to develop, strengthen and optimise the Virtual Ward admission and discharge pathways that includes engagement with Emergency Acute Portals, Wards, Community Services, and GPs

Management of Virtual Ward staff including the management of the Virtual Ward Clinical Lead and the Advanced Clinical Practitioner(ACP)

Direct Management of the virtual ward beds to ensure that we are maximising and monitoring the agreed bed capacity in each geopgraphical area at any given time.

Taking the Lead, in working with referring services and the Unscheduled Care Co-ordination Centre in making decisions on accepting or declining patients onto the Virtual Ward.

Responsible for the case management and allocation of patients to the Virtual Ward clinical staffing that have been referred through to the Virtual Ward based on the level of acuity of the patient.

Liaising and engaging with key professionals, including Acute Medics, Senior Managers, Senior Clinicians, GPs, Community Senior Managers and Local Authority Leads to ensure that all are working to the agreed operating pathways for the virtual ward and achieving the same goal of patient clinical safety, admission avoidance and supported early discharge where possible.
