**Job overview**:
The community forensic team is recruiting for 2 band 4 senior time recovery workers to enhance the existing county wide provision.

This is a community-based role supporting the care coordinators, psychologists, medical team and social worker to deliver additional support and follow up in the agreed setting which may include patients place of residence to maintain mental health well being, improve independence and promote social inclusion. The team is based within the Police Hub in Bodmin but is county wide.

The forensic multi disciplinary team manages the caseload of those individuals requiring specialist care coordination and provides a consultation service and specialist learning disability and autism service.

The team works closely with in-county low secure inpatient services as well as those clients within the secure pathway in out of county provision.

**T**he post-holder will work across Cornwall and may be expected to travel county wide independently as well as occasional travel outside of the county.

**Main duties of the job**:

- Work as part of a multi-agency team, working alongside qualified Senior Mental Health care coordinators, psychologists, psychiatrists and social workers. The post holders will also be directly involved in working with the individual placement support worker working within the team from Pentreath industries.
- The post holder will be in regular contact with housing providers, other criminal justice providers i.e., Police and Probation the co located liaison and diversion team and both inpatient and community mental health services.
- To actively engage and work with several individuals with offending histories combined with a wide range of mental health problems, who may also have co-existing complex needs including homelessness, substance misuse, significant risk to self or others, are socially excluded and who at times may present with extremely challenging behaviors.
- To contribute to the support and treatment provided by the wider team working at times as a lone worker with established direction and supervision from professionals within the team.
- To provide practical support to clients, to access a wide range of agencies and services and to engage with community and maintain and promote recovery and independence.
