**Overview of the role**

**Key duties**
- Providing personal care - including bathing, toileting, dressing and help with feeding
- Medication administration
- Assisting and getting involved in residents' daily activities, hobbies or outings
- Following care plan guidance alongside Company policies and procedures
- Developing an open, honest, and considerate working relationship with residents
- Maintaining written daily records
- Attending and completing training, as required

**One day a week that will be assigned to you from management will be the role of Positive Intervention Practitioner. Key duties for this role are**:

- To promote and maintain the positive well-being of the people who live in services provided by Choice Care by assisting in the provision of Positive Behaviour Support Framework for individuals supported by the organisation.
- To uphold high standards of Positive Behaviour Support in line with policy and practice.
- To develop and maintain a special interest in recovery-based approaches and support for people with learning disabilities and / or mental health difficulties.
- Support the workflow and the needs of the assigned Service.
- As guided by the Clinical Lead and the Senior Practitioner to undertake assessment, formulation work alongside individuals in the context of their history and relationships. This will occur within a broad framework of Positive Behaviour Support and Recovery based thinking.
- To visit and co-ordinate on going behaviour monitoring of challenging behaviours in relevant CHOICE CARE assigned service.
- To assist in the development of interventions aimed to reduce the intensity, frequency and potential challenge of behaviours that are challenging to the individual and the service.
- To work with members of support staff to promote independence and the other outcomes as identified in the personalised Positive Behaviour Support Planners.
- To offer clearly specified individual and group work to people who receive Choice Care services under the supervision of the Clinical Lead and or the Senior Practitioner.
- To facilitate the smooth and efficient implementation of the Care Plan systems within the homes.
