The post-holder will be expected to undertake all the normal duties and responsibilities associated with a Clinical Pharmacist working in General Practice and the out of hours setting including: Develop, produce, and implement medicines policies and procedures. Develop, produce, and implement medicines management strategies to ensure compliance with CQC, QoF, legislation and local quality frameworks. Develop, produce, and implement clinical audit in relation to medicines. Incorporate the objectives of medicines optimisation into every day general practice, including the aims of national and local strategies.

Lead, plan, develop and support the introduction of new working processes within the services. Undertake clinical structured medication reviews and repeat prescribing when necessary. Reconcile medicines following discharge from hospitals and intermediate care, including identifying and rectifying unexplained changes, and working with patients and community pharmacists to ensure patients receive the medicines they need. Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g., those with medicine compliance aids or those in care homes).

Identify cohorts of patients at high risk of harm from medicines through practice computer searches, including risks that are patient related, medicine related, or both. About you Minimum of 5 years post-qualification experience An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare Experience of medication review, face to face and remotely Experience of repeat prescribing and prescribing for long term conditions within competency Experience and knowledge of developing and undertaking clinical audit Whats on offer? 6 Month fixed term contract £58,012 - £68,552 per annum, DOE Based in Litherland with travel required across Liverpool & Sefton Hybrid working optional NHS pension scheme 27 days annual leave per annum Casual dress working environment Ride to work scheme Long service awards
