An exciting opportunity has arisen for an enthusiastic and hardworking Practice Consultant to join The Northern Learning Disabilities Team based in North Norfolk. Covering North Norfolk and Broadland our service provides an integrated Health and Social Care approach to assessment and support in the community.

**About you**

Are you an experienced Social Worker or Practice Consultant with a strong commitment to strengths - based Social Work? Who has the skills to work in a multi-disciplinary setting and with complex situations?

In this role you will be a strong and motivated leader supporting the team with practice development and service improvement. You will be passionate about making a difference; and ready to embrace change.

You will have the ability to work under pressure, be well organised and able to effectively prioritise. You will take a lead role in the team including managing workflow and providing casework consultancy to Social Workers, Assistant Practitioners, and students.

You will need to have a positive attitude and enjoy finding creative and innovative solutions. You will need to have excellent communication skills, record keeping and keyboard skills. You should have a commitment to excellent customer service and be confident in strengthening Social Care practice.

**Norfolk County Council and Norfolk Community Health and Care Trust deliver together a single service for adults with a learning disability. We are an integrated Health and Social Care team providing specialist learning disability support to adults in the community.**

You would be joining an inspiring and dedicated team of Social Care practitioners who are focussed on supporting adults with a learning disability to live independent and fulfilling lives.
