This is an exciting opportunity to work for Wyre Enhanced Primary Care as a Neighbourhood Assistant in the community.

Our team is made up of community matrons, therapists, integrated care coordinators, health and wellbeing support workers and other neighbourhood assistants whilst also work closely with the District Nurses and other community teams.

The post holder will support the integrated community neighbourhood teams carrying out agreed clinical and administrative responsibilities. You will be working with patients with a variety of physical conditions in their own home and community.

When working alone in the community you will be required to have initiative, excellent communication skills, time management and will always have the person’s best interests at heart when delivering patient centred care.

A car driver with use of a vehicle is essential.

The post holder will work in the Community setting supporting the clinicians within the Neighbourhood Care Team, carrying out agreed tasks. The duties will be both nursing, therapy and clerical, aiming to provide appropriate support within an integrating team. The post holder will accept the responsibility for doing a quality job, aspire to achieve, share a corporate pride in achievements and strive to act as an ambassador for good quality.
