Care Navigator

Mount Pleasant Practice is looking for a skilled care navigator to join our knowledgeable and well-established care navigator team. Ideally you will have experience of using Emis and have an excellent approach to customer service.

Main duties of the job
- Conduct telephone and face to face contact with patients in a professional, friendly and helpful manner
- Care-navigate/signpost patients to the most appropriate health care professional ensuring appointments are allocated where required in line with practice protocol and procedure
- Accepting, issuing and handing out letters, completed forms, specimen bottles, etc.
- Dealing with requests for information.
- Greet and direct patients and visitors.
- Liaise with associated Health Care Professionals.

**Job Type**: Part-time

Pay: £11.44 per hour

Expected hours: 16 per week
