**About the role**

We have a great opportunity for a part-time **Palliative Care Support Worker** to join our Community Specialist Palliative Care Team. This post is to provide cover for maternity leave and would be offered as secondment opportunity or fixed term contract of 9 months.

**Hours**: Part-time, 15 hours per week worked (Monday to Friday 9 am to 5 pm).

Weekday working days may vary, a rota is produced up to 8 weeks in advance.

Our Palliative Care Support Workers work as part of a multidisciplinary team of doctors, specialist nurses, physiotherapists, occupational therapists and supportive care (spiritual care practitioners, social workers, complementary therapists) to ensure patients of north Derbyshire get the support they need. This includes helping to manage difficult symptoms and providing advice when needed. We want our service users to live as comfortably as possible and understand what choices they have regarding our services and the support we provide.

The Palliative Care Support Worker service is available Monday to Friday from 9 am until 5 pm.
- _The main responsibilities include:

- Contributing to the assessment, monitoring and support of patients and carers in their own homes, identifying physical and/or psychological symptoms, they will liaise with the appropriate health and social care team, completing referrals to other agencies as required. This will be through face-to-face and telephone contacts.
- Having responsibility for a designated caseload, within the locality area.
- Delivering direct nursing care for patients.
- Maintaining accurate and effective patient records and documentation
- Demonstrating excellent communication skills, within the MDT and wider health care system.

**About you**:

You will have outstanding communication and organising skills, work well as part of a multi-disciplinary team and be committed to making a positive impact on the lives of patients, families and colleagues alike.

Skills and experience required.
- Able to demonstrate an understanding of the needs of terminally ill people
- Confident in documenting care delivered
- Motivation and commitment to personal and professional development
- A commitment to Ashgate’s values
- Having a driving license and access to a car
- Clear communication skills, to work as part of team or individually
