Enhanced Clinical Practitioner

We are looking to recruit a highly skilled Enhanced Clinical Practitioner to deliver care to the residents of Purbeck. An opportunity to be involved in a truly integrated way of working to deliver high quality health care.

You will be joining a multi-disciplinary service to provide rapid response and admission avoidance to patients, working with the team in service delivery, triaging and prioritising visits to achieve overall objectives.

The Enhanced Clinical Practitioner will be based in the Purbeck Hub, Wareham. You will join an excellent team of Practitioners, who are supported by GP's, a hospital consultant and Long term Condition HCA's. The Hub offers a truly integrated service with Social Services supported by Health and Social care co-ordinator's, rehabilitation services, Community Nursing teams, GP's, pharmacists and a phlebotomy service. We have close links with our community hospital in Swanage where we have access to step up beds.

You will provide clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and decision-making skills. Your role will be community based but will also support the community hospital supporting the transfer of community patients.

Hours will be part time at 30 per week, working 8.30am - 4.30pm or 9am - 5pm but as the service expands shifts will cover 8am - 8pm with a possible future weekend rota as the networks develop.
