**12 months fixed term / secondment opportunity (see attached form)**
Are you looking to be part of the development and leadership of a new team, whose purpose is to avoid inappropriate admissions to hospital. Are you a practitioner with a special interest in challenging behaviour, Autism and mental health wellbeing for people with learning disabilities. If you are creative, compassionate and passionate about enabling people to have a voice and choice in their life, we would love to hear from you.
The role includes working directly with people who use our services, their carers, colleagues and other service providers in an interdisciplinary way, to assess service user's needs, devising care plans, delivering care and support as well as evaluating outcomes.
The successful applicant will be based in Coventry however the role requires travel across Coventry, Warwickshire and Solihull, flexibility is also required over a 24-hour period.
