Hunter Mental Health have an exciting opportunity for a great Community Pyschiatric Nurse in a community mental health team. The role will be responsible for delivering ongoing mental health assessments for service users with severe and enduring mental healthneeds living in the community, in a northwest Wiltshire service. This is a large mental health trust who pride themselves on the safe and excellent mental, physical and social care that they provide to patients across all ages. improving their health and wellbeing.

Key responsibilities of the Community Pyschiatric Nurse role as follows:

- Care co-ordination of a complex caseload of service users with psychotic illnesses
- Fulfilling the CMHT duty worker role on a rolling rota
- Implement risk assessments and care plans
- Ensure the correct administration and monitoring of medication
