Specialist frailty pharmacist: Home First Response Team/Primary Care Pharmacy Team

1 WTE 12 month fixed term secondment

This is a unique opportunity for an enthusiastic and self-motivated pharmacist to work across the new Home First Response Team at the front door of the RAH, and the primary care pharmacy team working with General Practice.

The home first response team is a new pilot service that supports the principles of providing care closer to home. It aims to reduce admissions to hospital via a new hub and spoke frailty team at the front door of the RAH known. This is a multidisciplinary team that delivers interventions that enable patients to maximise their health and independence through a coordinated approach. The Team will act as a conduit between acute and primary care to ensure effective communication and care planning to optimise the patient pathway.

The broad aims of the service are:

- Professional assessment and support decision making around optimum place of care
- Service provision to address all levels of need, complexity and vulnerability
- Shift in service delivery towards local community provision.
- Development of patient centred pathways.

You will provide a safe and effective specialised clinical pharmacy service to this team and patients living with frailty, including working autonomously as an independent prescriber.

This posts will be required to work across traditional service boundaries as part of the multi-disciplinary team to support the delivery of care to frail older adults, liaising with and referring to health and social care colleagues to provide comprehensive patient management and optimise patient outcomes, including risk assessment for complex patients.

This post is split between the HFRT and the pharmacy HSCP team (50:50), providing close links across all sectors to improve the transitions of care for patients. The HSCP role will involve support to high risk patients and medicines as well as elements of prescription management. As a key member of the Prescribing Team you will have an important role in promoting and implementing cost effective and evidence based prescribing within specific GP practices and Care Homes across Renfrewshire HSCP. This role will involve working with GPs and the primary care multidisciplinary teams to improve medicines management and deliver excellent patient care. This role will involve delivery of the pharmacotherapy service as part of the GP contract and will provide an important function in supporting patients with their medicines.

This post requires excellent communication, negotiation, organizational and IT skills along with the ability to influence and affect change. Applicants should preferably be an independent prescriber confident in delivering autonomous clinical care.
