To provide dedicated support for patients on the two week wait prostate cancer pathway in line with national Cancer Waiting Time standards.
- To Ensure the management of patient is efficient and effective, taking into account the individual needs of patients and the requirements of the service. To contribute to the achievement of the National Cancer Waiting Times targets.
- To play a pivotal role in supporting the Multi-disciplinary Team (MDT) in coordinating services to deliver the specified outcomes and targets of the Cancer Plan.
- To facilitate the patient pathway by co-ordinating and proactively tracking the timeliness of patient treatments within the National guidelines.
- Close and effective liaison with different departments and neighbouring Trusts is an essential aspect of this role. Excellent organisational communication skills together with high levels of accuracy are therefore a key requirement for this post.
- Provide the outpatient and theatre rota co-ordination, resolve data quality issues and monitor the RTT position thereby enabling the 18 week RTT target to be achieved by the Care Group.

All work must be undertaken ensuring strict compliance with all Trust policies and procedures. Line Management of Oncology administration team. Ensure appraisals and mandatory training for team are up to date and provide support when required and as directed by Management.

We are looking for an enthusiastic and committed Navigator to work within the Oncology service line cluster. The Navigator role will be to provide dedicated support for patients on the two weeks wait cancer pathway in line with national Cancer Waiting Time standards.

Available as a fixed term contract or secondment opportunity (with current line manager's agreement).

Coordinate care by providing a single point of access for patients referred on the two week wait (2ww) prostate cancer pathway

Track patients referred on the 2ww pathway to improve flow

Provide basic telephone advice prior to diagnostic appointments and refer on or sign-post to other sources of support

Coordinate the necessary assessments, appointments or investigations to fast track people into the most appropriate clinic

Demonstrate the ability to recognise and respond appropriately when faced with an unexpected issue

Escalate any delays within the diagnostic pathway

Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit

Administrative Duties

Chasing / tracking results

Booking/amending appointments

Ordering patient information and updating as necessary

Audit / research

Record patient outcomes on Taunton & Somerset system
