The Proactive Ageing Well Service is a multi-disciplinary triage and case work team (known as the PAWS Team) contacting patients via phone and home visits, carrying out Frailty Screening with the support from a range of different health professionals.

The role of Community Navigator will be to triage and assess people’s support needs, provide case work and social prescribing, screen patients to identify levels of frailty and work with the wider PAWS teams on the day-to-day delivery of the service.

The Post will be collocated and work in close partnership with the PAWS team members, consisting of a Physiotherapist, Pharmacist, Specialist Nurse and the Navigator; supporting the identification of people living with moderate frailty and making an assessment to identify their health and social needs. These are predominantly around risk of falls, polypharmacy and other medicine related issues and social isolation.

This role is well suited to an individual with experience of directly working one-to-one with patients/clients, families and carers, to plan and help people resolve practical issues with Housing, Finances, Employment alongside wider social prescribing support, such as keeping active and connected.

Your role involves working with people who may be experiencing complex issues, so you will need good listening and communication skills and be able to build effective partnerships with a wide range of organisations and community groups.

You’ll need excellent IT and organisational skills, be adept at seeking out the information for relevant community services and have a tenacity to help people achieve sustainable improvements in their wellbeing.

Contract length: 10 months
