Discharge Co-ordinator

As part of our ESHT Rehabilitation Strategy, we have an exciting opportunity to expand our therapy workforce across our Intermediate Care Services at East Sussex Healthcare NHS Trust. Our Intermediate Care Services cover the Irvine Rehabilitation Unit and Rye Memorial Care Centre.

We are recruiting to the role of a Discharge Coordinator to join the intermediate care service to work alongside our multi-disciplinary team supporting patients as they continue their rehabilitation journey. You will support the development and implementation of the Rehabilitation Strategy - it's an exciting journey to be a part of.

We are looking for a passionate person who works well in a team environment, is highly motivated and committed. The posts involve a mixture of operational and administrative duties. You will be working alongside members of the multi-disciplinary team (such as Physiotherapists, Occupational Therapists, Speech and Language therapists, Dieticians, Nurses, and Social Workers) to deliver high-quality rehabilitation to the patients within the Intermediate Care Units of East Sussex Healthcare NHS Trust
