Contract length - 6 months

Working hours - 9am - 5pm Monday - Friday

**What you’ll be responsible for**:

- The post holder will be responsible for the formulation, execution and evaluation of a care plan for a designated number of patients who may present with some complexity within a multidisciplinary team.
- The post holder will ensure that each individual plan is developed in collaboration with the patient and informed by the wider multidisciplinary team.
- The post holder will operate within the principles of recovery and as such support each patient to develop a Wellness Recovery Action Plan to promote empowerment and self management thus reducing dependence on Secondary Mental Health Services and enabling a planned integration into local community facilities and the discharge as part of the patient’s recovery plan.

The role will involve, but not be limited to you performing the above.

**You’ll have the following qualification/experience**:

- A minimum of 6 months experience as a Mental Health Care Assistant (Qualifications required to accompany individuals with 6-12 months experience)
- Studying as a Student Nurse with at least 12 weeks’ clinical placements, studying towards a Mental Health Nursing degree

*Depending on the Trust, you may require variable additional certification and skills.
