Wolverhampton health visiting service is undergoing exciting service development and expansion resulting in the creation of some new posts, as part of this we are looking for enthusiastic and dynamic Health Visitors to join our 0-19 Health Visiting Service.

Are you an inspirational and highly driven experienced Health Visitor or newly qualified Health Visitor who has exceptional leadership and clinical skills and want to make a difference to our health visiting workforce and families of Wolverhampton? We have vacancies both in the Health Visiting teams and our single point of access which has recently expanded to include Health Visitors as part of our improving access to support our families and professionals

Car owner/driver essential and you must be able to meet the travel requirements of the post.

To be professionally accountable for the maintenance of standards of professional practice as set by the appropriate regulatory body applicable to your profession or role.

To deliver and manage programmes of care to a designated caseload/population ensuring efficient and effective use of resources to improve individual and family health outcomes.

Providing a universal preventative healthcare service to young children and families through the healthy child programme

Working with external stakeholders to ensure children and families have access to and benefit from partnership plus services

As a Health Visitor you will individually and within a team be accountable for the management and delivery of programmes of care for a designated caseload/population.

Ensure the delivery of clinically effective practice through the effective utilisation and integration of evidence based practice and implementation of evidence based standards of care, policies, procedures and protocols.

Ensure a culture of continuous quality improvement through the use of audit, patient/family/stakeholder feedback and reflection on practice by self.

Ensure the identification of the health needs of families and communities, using appropriate methods and tools.

Promote a clean and safe environment for staff, children and families by ensuring compliance with legislation, policies and protocols including health and safety, healthcare associated infection prevention, clinical and information governance, including risk management, critical incident reporting and root cause analysis.

Ensure effective communication with the multi-professional team, statutory voluntary organisations and other partners to ensure all care given is appropriate to the child and family and to prevent duplication of services.

Ensure a high standard of care and record keeping in accordance with Nursing and Midwifery Council, national legislation and local standards.

Report incidents and near misses promptly and appropriately and take effective action to minimise future risk.

Ensure an effective and efficient child and family’s journey by planning and co-ordinating the episode of care including the smooth transition to other settings, promoting effective communication with interdisciplinary and interagency teams as required.

Identify vulnerable families and children ‘at risk’ and participate in safeguarding activities, looked after children reviews, child in need and CAF’s as appropriate to promote positive health and prevent deterioration of the child and family’s health and wellbeing.

Assess and interpret clients conditions, deliver and evaluate health visiting interventions. Influence clinical decisions and monitor the quality of child and family care provided through clinical knowledge relevant to own field of practice, underpinned by theory and experience.
