**Job Purpose**:
The Community Connections project is funded through the Scottish Government’s Social Isolation and Loneliness Fund which is managed by Impact Funding Partners. The project will support care experienced young people (16-24 years) at Kibble and in the surrounding locality who are preparing to live/have recently transitioned to independent living; and/or young people who have fallen into addiction; and/or young parents. We will support young people to connect with their community, helping to address feelings of isolation and loneliness, and build stronger social connections.

Sitting within Kibble’s Housing Support service, the Support Worker will help young people connect to community life through a programme of tailored support.

**Main Duties**:

- To work alongside colleagues at Kibble and connect with organisations out with to identify referral routes for support.
- To meet with potential beneficiaries, discuss their needs, and prepare a support plan with them which will connect them to their community. Regularly check-in with young people.
- To regularly review and monitor the support plan with the young person, ensuring the support required reduces over time, helping them to eventually successfully live independently.
- To ensure young people are able to access services in their community e.g. register with a dentist, doctor, learn how to use a library etc.
- To help young people to acquire daily living skills and to help prepare them for moving to permanent accommodation where appropriate.
- To encourage and support young people to take an active part in activities within their community.
- To contribute to reviews as part of a wider multi-disciplinary team around each young person, where appropriate.
- To link young people to support groups/organisations in their community and help them to attend.
- To assist with the completion of statistical returns, providing information for report writing as required to the project manager and funding team.
- To build a database of other statutory and voluntary organisations to ensure support continues to be accessible beyond the lifespan of the project.
- To support the general welfare of the clients.
- To comply with Kibble’s Health and Safety policies.

**What qualification and experience are we looking for?**
- Minimum of twelve months relevant experience working with young people with care experience.
- Possess a full driving licence and be willing and able to drive provided transport.
- Take a flexible approach to work - shift work will be required in order to support the needs of young people.
