As the Community Links and Transport Assistant you will help facilitate the activities at the Local Link clubs as well as act as an escort on the minibus, providing safe transportation to the those utilising it.

You will be a wellbeing champion, promoting the Local Link clubs and goal-based befriending across Monmouthshire, ensuring all older people are aware of our offering which promotes wellbeing, reduces social isolation and acts as a support network for older people. You will work closely with older people, empowering them and building their confidence to take over the running of clubs, therefore making them self-sustainable.

You will assist with sourcing new venues, activity facilitators and opportunities to create Local Link clubs based on direct feedback from older people living in Monmouthshire. You will assist in accepting new referrals into the service, completing individual goal-based support plans and risk assessments and arranging the coordination of referrals to a Local Link club or befriender. You will need to maintain accurate records of clients, activity income and expenditure in line with Organisational cash handling procedures and risk assessments of club venues. You will support the Wellbeing and Development Coordinator with maintaining and providing accurate qualitative and quantitative data.

You will manage your work time effectively, have a passion for supporting older people and be flexible in order to meet service needs, including of working anti-social hours when required.

**The ability to drive and have access to a D1 licence and your own vehicle for work purposes is essential to your role. You will also undergo an enhanced DBS check.**

**Main duties and responsibilities**
- To assist with sourcing and developing local clubs for older people where they can socialise and access information, ensuring risk assessments for venues and activities are completed
- To be responsible for the transportation of clients to and from the clubs, either driving the ACG minibus or acting as an escort, ensuring the safety and security of the older people
- To be responsible for recording the minibus mileage accurately after each working day and report any changes relating to the bus to the Development and Wellbeing

**Coordinator**
- To help plan and assist with delivering a rolling programme of activities at the local clubs with specific reference to health, security and wellbeing
- To assist with the running of our lunch and locality clubs when required, monitoring attendance and collating feedback to return to the Wellbeing and Development
- To interact with the service users attending the clubs and note and report any concerns or changes in the service users’ needs to the Team Leader.
- To make sure group sessions are organised safely and efficiently, ensuring that any necessary equipment or consumables are readily available
- To ensure risk assessments for venues and activities are completed and safeguards are implemented
- To be able to deal with any emergency arising during the group sessions
- To be a wellbeing champion, promoting independence and encouraging older people to become actively involved in the development of creating sustainable local clubs
- To assist with the provision of a monitoring service, offering short term support, information and guidance
- To visit potential clients and complete a comprehensive risk assessment and goal based support plan for each individual
- To maintain full and complete records, using a computerised database (Charity Log) in line with Organisational standards
- To routinely monitor new and existing clients through the completion of client support plan reviews, enacting any changes identified
- To be responsible for the collection of any income generated at the clubs, completing a payment collection record and liaising with the Finance Department
- To maintain confidentiality and understand data protection - GDPR regulations
- To ensure that safeguarding of clients is a key priority and complete any necessary internal and external procedures and actions
- To be prepared to undertake regular training as required under external and organisational regulations, including online training modules
- To form an active part of the Commercial Services Team and attend team meetings, individual meetings, external meetings, and supervisions
- To carry out any other duties commensurate with the level of this role across the organisations service delivery area
- To be willing to travel and work flexibly throughout the charities operational area

**Person Specifications.**

**Education and Training**
- A qualification in a related discipline or willingness to undertake within year 1
- A well-developed knowledge of IT either by qualification or experience in using databases, Outlook and Microsoft Office
- Admin/organisational skills and experience
- Driving license (D1) and access to a vehicle for work purposes
