Population Health Management (PHM) will transform the way we work across our new Integrated Care System, by using data to improve patient-centred care, reduce inequalities, plan improvements to services and make better use of resources. A PHM approach, including our PHM Development Programme (PHM DP), will become fundamental to how our Integrated Care System (ICS) works, from making evidence-led decisions on ICS priorities and use of resources to designing new models of care within Primary Care Networks.
During PHM DPs, health and care organisations in a specified area work together to use their combined data to design a model of care and intervention for an identified cohort of citizens. A series of Action Learning Sets takes a multidisciplinary group of operational, clinical and leaders through the PHM journey, working closely together on using data to identify a clear need and to design an intervention to help support those citizens identified. At the end of their programme, the identified cohort of citizens should start to see a direct benefit to their care and support.
