To provide dedicated support for patients on the two week wait prostate cancer pathway in line with national Cancer Waiting Time standards.

To coordinate care by providing a single point of access for patients referred on the 2ww pathway

To ensure smooth transition along the prostate cancer pathway

To provide support for the prostate CNS team and cancer services

We are looking for an enthusiastic and committed Urology Navigator to work within the Urology service line cluster. The Urology Navigator role will be to provide dedicated support for patients on the two week wait Prostate cancer pathway in line with national Cancer Waiting Time standards.

Available as a fixed term contract or secondment opportunity (with current line manager's agreement).

Coordinate care by providing a single point of access for patients referred on the two week wait (2ww) prostate cancer pathway

Track patients referred on the 2ww pathway to improve flow

Provide basic telephone advice prior to diagnostic appointments and refer on or sign-post to other sources of support

Coordinate the necessary assessments, appointments or investigations to fast track people into the most appropriate clinic

Demonstrate the ability to recognise and respond appropriately when faced with an unexpected issue

Escalate any delays within the diagnostic pathway

Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit

Administrative Duties

Chasing / tracking results

Booking/amending appointments

Ordering patient information and updating as necessary

Audit / research

Record patient outcomes on Taunton & Somerset system

COMMUNICATIONS & WORKING RELATIONSHIPS

Reporting and documenting all relevant actions and/or patients’ needs

Communicates changes, with other health care professionals, of patient condition/ needs as they occur

Provide delicate, sensitive and/or complex information concerning patients’ condition, which requires persuasion and reassurance skills. Some patients have special needs or learning disabilities

Engage, interact and teach carers’/families to be actively involved in patients’ care
